Information & Nomination Form

Friday, January 25, 2008. In order to insure fairness, nominations received after the

Intent: To recognize individuals who consistently demonstrate the highest degree of interest, personal dedication, and service in a voluntary capacity to the improvement of health, welfare, and education in Summit County. It is the highest volunteer honor given by this United Way.
Eligibility:
Any individual who displays the characteristics consistent with the intent of the
award are eligible. Nominees should live and/or work in Summit County and should have demonstrated significant service to a variety of human service
agencies or programs that directly impact social service issues for which the entire community benefits. Due to the active, ongoing nature of the award and its intent, candidates nominated posthumously will not be considered.
Preference is given to nominees with a history of volunteer service to United Way and/or affiliated agencies.
Note:
Only nominations received this year will be
considered. If someone you nominated previously has
not received the DSA, we encourage
you to re-nominate that individual.
The award will be presented at United Way of Summit
County's 89th Annual Meeting to be held Wednesday, April 2, 2008.
